
Rotary Club Bulletin: Special Report on Healthy Ears and Healthy Aging

Introduction
At our latest Rotary Club meeting, we were privileged to host a compelling presentation by Ellen, a seasoned audiologist with a decade of experience, and Theresa, her colleague with 17 years in hearing aid management. Their talk, "Healthy Ears and Healthy Aging," delved into the critical connection between hearing health and overall well-being.
Ellen and Theresa's presentation underscored the profound impact of hearing loss on various aspects of health, from cognitive function to balance and sleep quality.
The Anatomy of Hearing
Ellen began by explaining the structure of the ear, which is divided into three main parts:
- Outer Ear: The visible part of the ear and the ear canal up to the eardrum.
- Middle Ear: Contains three tiny bones and an air-filled cavity connected to the throat.
- Inner Ear: Hosts the cochlea for hearing and the vestibular system for balance.
Types of Hearing Loss
- Conductive Hearing Loss: Typically occurs in the outer or middle ear and is often treatable. Common causes include earwax buildup and infections.
- Sensorineural Hearing Loss: Involves damage to the inner ear or auditory nerve and is usually permanent, necessitating hearing aids.
The Broader Impact of Hearing Loss
- Cognitive Decline and Dementia - Research, notably a 2015 study by The Lancet, has shown that untreated hearing loss significantly increases the risk of cognitive decline and dementia. Ellen emphasised that individuals with untreated hearing loss are five times more likely to develop Alzheimer’s or dementia. This is due to auditory deprivation, where reduced sound signals to the brain impair cognitive and memory functions. Hearing aids are crucial in maintaining brain health by providing continuous auditory stimulation.
- Balance and Falls - Hearing loss is closely linked to balance issues. The cochlea, responsible for hearing, is also connected to the balance organ. Disruptions in hearing can affect balance, making individuals with hearing loss three times more likely to experience falls. This increased risk is attributed to the cognitive load required to compensate for hearing loss, leaving fewer resources for maintaining balance. Hearing aids help improve auditory input, thus enhancing balance and spatial awareness.
- Sleep Quality - Hearing loss negatively impacts sleep quality. This leads to increased daytime sleepiness, frequent awakenings and difficulty falling asleep. This relationship is bidirectional. Chronic sleep issues can lead to hearing loss due to insufficient oxygen supply to hair cells, and hearing loss can disrupt sleep as individuals remain in lighter sleep stages to stay alert to potential dangers. Effective management of hearing loss can improve sleep quality.
- Tinnitus - Tinnitus is characterised by ringing or buzzing in the ears. This affects 30% of people and is especially prevalent among those with hearing loss (80%). Tinnitus is a phantom sound generated by a lack of auditory stimulation. Stress can exacerbate tinnitus, creating a cycle of worsening symptoms. Although there is no cure, hearing aids are the most effective management tool, masking phantom sounds and providing relief.
- Interactive Demonstration - Theresa led an interactive activity to demonstrate the impact of hearing loss on balance. Members were asked to perform simple balance exercises, highlighting how cognitive resources are taxed by hearing loss, making balance more difficult to maintain. This practical demonstration effectively illustrated the importance of auditory health for physical stability.
